"This area still looks a little thin to me, and one side seems different from the other. Can't I just get a touch-up now?"

The short answer is: what you see right after fat grafting is not the final result. The transferred fat goes through an engraftment process during which both volume and position continue to change. In the first two to four weeks, swelling and engrafting fat are intermingled, making it impossible to read the true outcome. It isn't until around six to eight weeks that engraftment stabilizes and actual volume can be properly assessed. Performing a touch-up before this stabilization point means potentially adding more fat on top of grafts that would have settled just fine on their own — essentially doubling up when it isn't needed.

FAMI (Fat Auto-graft Muscle Injection) is a technique that precisely layers fat into both the muscle and fat planes. Each layer requires adequate time to establish new blood vessel connections and achieve stable engraftment. Why Is a Retouch Before Engraftment a Problem?


Performing a retouch before the fat has fully engrafted can lead to a number of complications.

Adding more fat on top of grafted fat that is still in the process of engrafting can result in excessive volume once the final result settles. When the swelling subsides, you may be left with far more volume than anticipated.

Damage to Existing Grafted Fat

When additional fat is injected near tissue that is already undergoing vascularization, cannula movement can physically disrupt and damage the engrafting tissue.

Inaccurate Assessment

When swelling and engraftment are still mixed together, it is impossible to accurately determine which areas are truly lacking volume. Planning a retouch based on an unreliable assessment can actually compromise the final outcome.

Interference with Tissue Recovery

Re-stimulating the tissue while it is still in the healing process can slow down overall recovery and prolong unnecessary inflammatory responses.

For these reasons, Cellon Clinic does not evaluate the need for a retouch until at least 8 weeks (approximately 2 months) after fat grafting. This waiting period is not simply about patience — it is a medically necessary interval to ensure the accuracy of the final result.

Why the 2-Month Mark Matters So Much for FAMI Fat Grafting


Unlike conventional fat grafting, FAMI (Fat Auto-graft Muscle Injection) delivers fat directly into the facial muscle layer — and that distinction makes the timing of any touch-up far more critical.

FAMI precisely injects fat into the facial muscle layer rather than the subcutaneous layer, creating natural-looking volume in exactly the right position. Because the muscle layer has a relatively rich blood supply, engraftment rates tend to be higher — but the fat also needs time to settle as the muscles move and adapt.

What Should You Do During the 2-Month Wait?


The waiting period doesn't mean there's nothing you can do. In fact, how you spend this time can have a significant impact on your final fat survival rate.

For at least the first four weeks after the procedure, avoid pressing hard on your face or getting facial massages. Alcohol is also best avoided for the first two weeks, as it affects blood flow and can interfere with healing.

Your sleeping position matters too. For at least two to three weeks post-procedure, avoid lying face-down or putting direct pressure on the treated areas. Sleeping with your head slightly elevated helps reduce swelling and supports stable fat engraftment.

Q It's been 2 weeks since my fat grafting, and the volume seems to have almost disappeared. Does that mean the grafting failed?

Not necessarily. The 2–4 week mark is when initial swelling subsides, which can make the volume appear to drop significantly. Because the absorption of non-grafted fat and the natural reduction in swelling happen at the same time, many patients feel like "everything is gone." A proper assessment of actual graft survival should be done at 6–8 weeks.

Q One side looks different from the other. Could asymmetry have developed? Should I do something about it right away?

Early asymmetry is very common. It's usually a temporary difference caused by swelling subsiding unevenly on each side. The right approach is to wait until after 8 weeks, when the graft has stabilized, and then consult with your provider to determine whether a retouch is needed. Acting too soon can actually lead to a more unbalanced result.

Q Does PAMI fat grafting require a longer retouch window than regular fat grafting?

Because PAMI involves injection into the muscle layer, facial muscle movement can influence graft survival — so full stabilization may take a bit more time in some cases. The baseline guideline is the same 8 weeks (approximately 2 months), but the exact timing is assessed individually based on the area and injection depth. Q For a retouch, is frozen fat used, or is fresh fat harvested again?
